- $10.00 Gift card available for each of the 6 (1mo, 2mo, 4mo, 6mo, 9mo, 12mo) WCEs by age 15 months (maximum of $60.00 in gift cards)
- $25.00 Gift cards available for completing the following:
- Annual WCEs from age 3-21
- 2 flu vaccines by the age of 2
- HPV (Gardasil) series completed before age 13 (needs to do between ages 11-12 for gift card)
- Annual dental visits
- Annual Chlamydia screening for 16–24-year-olds

Joining Healthy Rewards is optional. Once enrolled, you must complete the first preventative care activity. Once completed, a Healthy Rewards card will be mailed home and you will then be able to start earning rewards.
For members younger than 18 years old, rewards are added to the main household card.
